Reach and engage assigned members telephonically, in person and / or through the member's PCP. Some in-person contacts with each assigned member is required. Establish a trusting relationship with members and work with the member to assess the barriers that are prohibiting the member from seeking needed care. Connect members with physicians, provider offices, community resources and / or clinical services as appropriate. Collaborate and communicate with other members of the member's care team to improve the quality and efficiency of health care delivery and improve the member experience. Serve as the member's advocate and facilitator in overcoming identified barriers to care, e.g. connecting members with community resources, assisting members with transportation, locating translators. Assist members in understanding and complying with treatment recommendations from their PCP / Specialist, e.g. getting and taking prescribed medications, following special diets. Document all activities in the appropriate clinical management system.Positions in this function (with the exception of Healthcare Services Coordinator) require education / certification / licensure other than an RN, such as BSW, MSW, Respiratory Therapist, Dietician, Nutritionist, Occupational Therapist, etc. Positions are responsible for providing clinical and medical management services, including case management, health assessments, interventions, and discharge planning. May identify, coordinate or provide appropriate levels of care under the direct supervision of an RN or MD. This function does not include Health Coach, Health Educator and Health Advocate roles that do not require an RN. Those roles are found in the Wellness Coach Management function. Required Qualifications:High School Diploma or GED Must have resided within the local community for 2+ years.Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated areaExperience with outbound calls to members, Physician's, Pharmacy, Providers or similar medical facilityAbility to create, copy, edit, send and save utilizing Microsoft Word, Microsoft Excel and Microsoft OutlookMust live in Coshise County, AZ Preferred Qualifications:Bilingual (any language) Bachelor's Degree (or higher) Careers at UnitedHealthcare Community & State.
