In the Master Level Social Worker (LMSW) role, you will work directly with individuals, their providers and caregivers, and community-based organizations to identify available services to close medical, behavioral, functional, and social gaps in care for individuals with complex medical needs as well as those transitioning from acute inpatient settings to a community-based home. You will work directly with individuals, medical providers, and community organizations to identify services and resources to close medical, behavioral, functional, and social gaps in care for persons with complex medical needs as well as those transitioning from acute inpatient settings to a community-based home. In addition, and in conjunction with a multidisciplinary team, the Master Social Worker will provide consultation on, and assessment of, behavioral health issues impacting chronically ill and high- risk patients in a variety of environments including private homes, assisted living facilities, nursing homes, skilled nursing facilities, and / or hospital settings. Primary Responsibilities: Work directly with individuals and a multidisciplinary team to provide consultation, assessment, intervention, and education/training of behavioral health needs and service Assist staff in identifying and implementing effective communication and engagement strategies for working with persons with significant mental/behavioral health issues Identify, refer, and coordinate available community resources to address non-medical needs and ensure patient access to services Identifying solutions to non-standard requests and problems Clearly summarize pertinent clinical information in the patient record Communicate effectively with medical, professional, administrative and other personnel to present information in clear written and oral format Share clinical expertise with medical staff to ensure continuity and quality of care Participate in collaborative multidisciplinary team meetings to optimize clinical integration, efficiency, and effectiveness of service delivery
Required Qualifications:Licensed Master's degree-level clinician in Psychology, Social Work, Counseling or Marriage or Family Counseling, or Licensed Ph.D. Licenses must be active and unrestricted Strong written and verbal communication Ability to work in a self-directed manner, meet deadlines, and be flexible in your work day Ability to work with diverse care teams Ability to think creatively and comfortable being hands-on in a fast-paced, entrepreneurial environment Knowledge of, and experience with, behavioral health services, psychotropic medications, and therapeutic interventions to manage symptoms. 2+ years case management experience 2+ years post-graduate experience providing therapeutic interventions for individuals with mental / behavioral health conditions as a licensed clinician Ability and willingness to travel up to 50% for face to face meetings within the TN market Must have access to high speed internet for home office set up Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated areaPreferred Qualifications:Experience working with the Medicaid population Experience navigating complex social services systems Experience in managed care Experience using an EMR system (eCW / CareOne) Careers at UnitedHealthcare Community & State.
