This is the place to do your life's best work.(sm) As a Community Based Level of Care (LOC) Assessor, you will work with a diverse population group including individuals with mental health needs, elderly, those with a brain injury, intellectually disabled or other physical health needs. As a LOC assessor your primary role is to assess the individuals strengths and areas of supported needed. The result? Individuals will be able to have access to the supports they need to try to remain living in the least restrictive, most cost effective environment. We are hiring assessors in all of these areas and this position will require 75% travel within that city and surrounding areas:Davenport, Decorah, Des Moines, Dubuque, Fort Dodge, Iowa City, Mason City, Sioux City, and Waverly Primary Responsibilities: Visit Medicaid members in their homes and / or other settings, including community centers, hospitals or providers' offices Engage members face to face to have a discussion about their health Review available member service records and relevant documentation (e.g., utilization history, functional level, stratification information, current plan of care) Conduct comprehensive member assessment that includes bio - psychosocial, functional, and behavioral health needs Utilize interviewing techniques and active listening to collect and retain member information and incorporating responses as they are presented to complete assessment Identify when supplemental assessments are needed and conduct supplemental assessments Identify member service needs related to health concerns Identify urgent member situations and escalate to next level when necessary Research member - related health information (e.g., medications, clinical records, policies, benefit information) using internal and external systems Use desk top computer applications (e.g., Excel, Word, Outlook, Office Communicator) to summarize information (e.g., reports) or communicate member information to others Document information (e.g., assessment information, member interactions) in relevant computer systems Navigate multiple applications at once to obtain and / or document member information Learn updates and changes to computer systems / applications and apply to navigate applications effectively Execute and meet performance goals within a virtual environment
Required Qualifications: Bachelor's degree in a healthcare or social work related field 3+ years of clinical or case management experience in a hospital, acute care, home health / hospice, or direct care environment A diverse background working with individuals with disabilities such as mental health, elderly, brain injuries, developmental and intellectual disabilities and physical disabilitiesComputer / typing proficiency to enter/retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications Reliable transportation to travel to member homes or other locations within service delivery areaPreferred Qualifications: Current, unrestricted RN or SW license in the State Case Management experience Certification in Case Management (CCM) Home care / field based case management Medicaid, Medicare, Managed Care experience Experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs Bilingual skills Experience in Home & Community based or Long Term Care services delivery Experience with interRAI or SIS level of care assessmentsCareers at UnitedHealthcare Community & State.
