Join with us and start doing your life's best work.SMPrimary Responsibilities:Engage members face to face to educate about their healthReview available member service records and relevant documentation (e.g., utilization history, functional level, stratification information, current plan of care)Conduct comprehensive non - clinical member assessment that includes bio - psychosocial, functional, and behavioral health needsUtilize interviewing techniques and active listening to collect and retain member information and incorporating responses as they are presented to complete assessmentIdentify member service needs related to health concernsIdentify urgent member situations and escalate to next level when necessaryWork with clinical staff to engage the member in participating in the assessment process and collaboratively developing the plan of care based on the member's individual needs, preferences, and objectives.Work with members to identify barriers to accessing care and achieving person-centered goals, as well as work to close gaps in care.Identify member support systems available and incorporate into plan of careReview plan benefits and identify appropriate programs and services based on heath needs and benefitsWork with clinical staff to integrate health care and service needs into a plan / recommendations for member care and service. Transfer the member to clinical staff to manage clinical needs, as identified.Work collaboratively with the interdisciplinary care team to ensure an integrated team approachCollaborate with member to create solutions to overcome barriers to achieving healthcare goals-Identify relevant community resources available based on member needsRefer members to appropriate programs and servicesFacilitate member choice of preferred providerAdvocate for individuals and communities within the health and social service systems;Role assists in coordinating health services and does not write care plansHelp members connect with transportation resources and give appointment reminders in special circumstances. Transporting members is strictly prohibitedFollow - up with members via phone calls, home visits and visits to other settings where members can be found
Required Qualifications:High School Diploma / GED or higher Knowledge of Medical Terminology75% travel to locate members within the 60 miles radius Ability to create, copy, edit, send & save utilizing Word, Excel & Outlook1+ years Medicaid / Medicare experience OR 1 year + as a Certified Community Health Worker OR 1+ year experience working in the community / community programs OR 1+ year working as a case manager1+ years telephonic customer serviceAccess to reliable transportation that will enable you to travel to client and/or patient sites within a designated areaPreferred Qualifications:Completion of Community Health Worker Training programAssociate's Degree (or higher)Previous employment in a medical office, hospital or clinical or other provider environment Bilingual Careers at UnitedHealthcare Community & State.
