The AIDS Foundation of Chicago (AFC) mobilizes communities to create equity and justice for people living with and vulnerable to HIV and related chronic diseases. Our work focuses on improving health equity, preventing new cases of HIV, serving as a collaboration and knowledge center, and being a bold voice for change.
We are seeking for an individual to serve as our Medical Case Manager. The Medical Case Manager will provide a range of client-centered, confidential services that link clients with core care services including: medical, dental, mental health, substance use, and supportive care services including legal, medical transportation, emergency financial assistance, food and housing services. The goal of medical case management is to ensure viral suppression by taking care of any barriers to care services preventing a client from being undetectable. The Medical Case Manager will provide treatment adherence counseling to ensure readiness for, and adherence to complex HIV/AIDS treatments.
Principal functions and responsibilities of this position include the following:
Assessments & Service Plans
Conduct face-to-face assessment and reassessments every six months;
Create care plans that match the identified needs of the client;
Ensure that all clients referred to services are eligible prior to the date of service.
Provide care services linkage and re-engagement support to clients;
Conduct home visits as needed to RW and HOPWA clients who have disengaged from care;
Conduct outreach services as needed;
Refer clients to appropriate services highlighted in the care plan;
Screen for income supports for benefits and entitlements to advocate on behalf of client;
Complete emergency housing applications for eligible clients;
Complete payment requests as needed for copays and other out of pocket costs;
Document encounters using designated client database accurately and in a timely manner;
Develop and maintains professional relationships with social service, health and governmental agencies;
Participate in continuous quality improvement efforts;
Participate in regular supervision meetings, team meetings, in house training sessions (12) and conferences as needed.
Assist with agency-wide activities as directed, including Annual Meeting, AIDS Run & Walk, and others;
Protect organization's value and manage risk by keeping information confidential;
Perform other duties as assigned.
Associate’s degree in any human services field with at least 4 years of case management experience
Master’s / Bachelor’s degree in any human services field
Master’s / Bachelor’s degree in any non-human services field with at least 2 years of case management experience
A Registered nurse
Knowledge, Skills, and Abilities
Exceptionally verbal communication and active listening skills.
Strong attention to detail and the ability to provide efficient, quality service to both internal and external customers.
Solid administrative skills and knowledge of Microsoft Excel and/or other database programs.
Solid interpersonal skills along with the ability and willingness to respect and value the differences and perceptions of different groups/individuals.
Strong case management skills with basic knowledge of HIV/AIDS and understanding of HIPPA.
Solid adaptability/flexibility skills and ability to follow up with clients in a timely manner.
Strong time management skills along with problem solving skills and ability to quickly and accurately process multiple types of information and/or perform mutable tasks simultaneously.

Anyone can become infected w...ith HIV, but HIV and related chronic diseases disproportionately impact certain populations in distinct geographic areas. We can have the greatest impact on the HIV epidemic by focusing our efforts on those most-impacted by HIV, based on epidemiological data and unmet need:
• Young Black gay and bisexual men
• Transgender women of color
• Black women living in high-incidence areas
• Latino gay and bisexual men
Factors that drive the HIV epidemic and health inequities that we will address include:
• Sex work
• Drug use
• Lack of access to health care
• Immigration status
AFC will also work to meet the evolving needs of older adults with HIV and long-term survivors.
