Work in collaboration with home health patients, clinical management team, and home health RNs to provide home health care services that reflect the home health care agency philosophy and standards of care.Primary Responsibilities:Use a LPN level of medical knowledge to accurately and completely assess the client's physical, psychosocial/emotional and environmental condition and needs following specific physician orders.Respect confidentiality and maintain confidences as described in the Confidentiality Security Agreement that is signed by all employees. The ability to maintain confidentiality is a critical and essential component of this position.Exercise clinical knowledge and appropriate level of judgment in assessing clients' status and needs and respond appropriately. Refer or defer as appropriate. Higher level medical staff (e.g., RNs) are available as resources.Provide nursing care and interventions that are timely, appropriate, and in accordance with nursing standards of care and established policies and procedures.Assess the changing needs and condition of the client and communicate this information to the clinical management team, physician and other appropriate individuals, according to department policies and procedures.Respond to, coordinate, and document patient and staff incidents in accordance with existing policy and procedure and notify appropriate staff within designated timeframe.Document assessments, client/family response to nursing interventions and appropriate home health care functions at the time of the visit. Meet departmental standards and deadlines for timely completion of all required documentation and meet current agency productivity standards.Participate in staff meetings, case conferences and in-services.Work collaboratively with various functional departments, individuals, clients, and providers in accordance with the Mission, Vision, and Guiding Principles of the company to maintain and enhance RMHP's reputation as the leader in managed health care.This position requires travel. Requires independent, reliable, flexible, and on-demand, transportation at the incumbent's expense for travel between various locations and timely arrival and departure from various locations. If the employee chooses to satisfy this requirement by driving a vehicle, the employee must meet the requirements for Colorado licensure and company requirements for liability insurance coverage.
Required Qualifications:High School Diploma / GED (or higher)LPN License for the State of ColoradoRegistered or certified by the Colorado Department of Regulatory Agencies (DORA)CPR certification or the ability to acquire certification within 4 weeks of employmentExperience using a computer and Microsoft Office including Microsoft Word (create correspondence and work within templates), Microsoft Excel (data entry, sort / filter, and work within tables), and Microsoft Outlook (email and calendar management)Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated areaAbility to travel locally 100% of the timeAbility to work 8:00 am - 5:00 pm Monday through Friday with some evenings and weekendsPreferred Qualifications:1+ years of previous experience working as a CNAHospice or Home Health experienceNursing experience in a hospital or skilled nursing setting Physical Requirements and Work Environment:Ability to lift up to 40 lbs. frequentlyAbility to execute regular, reliable, and predictable performance of the essential functions of this job through a consistent ability to be physically and mentally present at the assigned work location, at the prescribed time, with the prescribed tools and with the availability necessary to successfully complete the essential duties described herein.Ability to work in home settings that include varying physical, social, and cultural environments.Ability to perform a variety of tasks that involve standing, walking, sitting, stooping, kneeling, bending and twisting, occasionally climbing stairs or using an elevator, possibly reaching chest high and overhead for materialsCareers at UnitedHealthcare Community & State.
