The Outreach, Prevention and Aftercare (OPA) team is dedicated to building community, hope, permanent connections, social and emotional well-being and opportunities for youth who are experiencing homelessness or at risk of homelessness. Our team provides youth with structure, support, and guidance in order to increase their self-sufficiency, health, independence and overall well-being. OPA participants receive engagement and support through street outreach, community engagement, and the agency’s Drop in Resource Center (DiRC). The OPA team provides participants with access to a variety of services including meals, transportation assistance, hygiene items, resources and case management services with a focus on increased self-sufficiency, safe & stable housing, and both educational & employment resources and opportunities.
The After Care & Stabilization Case Manager is responsible for working with the OPA Manager to support youth and young adults in need of aftercare and/or stabilization support while they are maintaining and/or transitioning in to housing. The After Care & Stabilization Case Manager develops relationships with assigned clients in order to provide adequate after care support and stabilization services. This position works collaboratively with the Housing team and external partners to achieve program goals. The After & Stabilization Case Manager is responsible for the direct case management of clients within the program and must demonstrate an understanding the mission and core values of Teen Living Programs. This position reports to the OPA Manager.
Provides appropriate transition sessions to youth by providing an overview of services offered through aftercare and stabilization program.
Provide appropriate referral and placement services to current and potential After Care clients.
Effectively completes all intake paperwork for After Care eligible clients and other TLP clients to ensure appropriate support and housing stabilization.
Maintain a collaborative working relationship with TLP Case managers to ensure appropriate documentation is completed.
The following duties are the essential functions of this position.
Functions as primary case manager for youth the assigned youth to receive aftercare services and/or youth in need of stabilization support.
Coordinates and conducts as appropriate, a comprehensive assessment process for clients.
Meets for individual case management sessions with After Care youth or youth requiring stabilization services on individual caseload.
Provides case management services, including but not limited to, addressing youths increased basic life skills, housing needs, education, employment, physical and mental well-being and makes referrals as needed, unless otherwise directed.
Assists youth with applying for various government assistance programs included but not limited to public aid benefit, health coverage, social security disability et, and support in obtaining any necessary documentation.
Provides basic case management support including but not limited to assessing client needs (i.e. Hygiene, clothes, housing, transportation passes, food support, external referrals),
Provides coaching support to ensure attendance and participation in appointments, meetings, and activities.
Distributes transportation passes to youth in accordance with the program transportation assistance policy
Conducts collaborative community case management to support clients with transitioning into identified housing match.
Documents all services clearly and thoroughly in HMIS, ETO, and paper files in a timely manner.
After Care & Stabilization Support
In collaboration with the program leadership team, oversees the overall smooth functioning of the Aftercare program.
Coordinates program planning, development, and evaluation of the After Care & Stabilization program.
Maintains documents regarding to client’s engagement with, daily activities and other records in compliance with agency and external policies, procedures and applicable licensing standards.
Participates in weekly supervision and trainings, with a positive attitude to identify opportunities to learn and accept/ implement feedback
Participates in program meetings, staff trainings, community meetings, client activities and other agency events.
Responsible for coordination of TLP’s client tracking project that involves the location of youth who have participated in TLP’s residential programs, collecting both client satisfaction and outcome data.
Participates in on-call rotation.
Performs other duties as assigned by the director of Programs.
Attends and participates in community engagement meetings/activities at the direction of the Director and/or Manager of OPA.
Provides appropriate youth engagement and intervention in all settings, (e.g., street, schools, partner agencies, etc.).
During community engagement events, dresses in a professional manner that is neat, clean and well-groomed in order to model the professionalism of TLP as an agency.
Conducts presentations for community partners, outlining the services provided by Teen Living Programs.
Has a valid driver’s license, a clean driving record and is willing and able to drive the agency vehicle when requested – Following background check, MUST be insurable under the agency’s insurance coverage, otherwise disciplinary action up to and including termination will occur.
Works as a part of a team to provide support and assistance whenever needed.
Prepares and submits all required forms and statistical programs. Maintain documents regarding participants’ cooperation, daily activities and other records in compliance with agency and external policies, procedures and applicable licensing standards in a timely manner.
Participates in program planning and evaluation.
Participates in ongoing training.

Must be able to climb stairs and short ladders on frequent, daily basis. May be required to occasionally lift up to 25 lbs.
· Education and Experience
Bachelor’s degree in social work or related program required.
At least 2 years of case management experience required, and at least one year in program management preferred.
Strong leadership and organization skills required.
