This is a Social Work position for the Information & Assistance Specialist. Position will be responsible for program and community resource education, and will act as a program liaison to the aging network.
Daily responsibilities will also include database entry and phone-based assessments related to program enrollment and resource referrals. Conduct comprehensive phone based assessments to determine social, financial, emotional and environmental strengths and limitations of potential clients. Coordinate the client's eligibility process for program assistance. Work with family members and volunteer caregivers to maximize available informal supports.
Provide feedback to AAA staff regarding any problems or concerns with service delivery from purchase of service providers. Establish and maintain liaison with hospital discharge planners, human service providers, home care agencies, long-term care facilities, Department of Human Services and other referring agencies. Provides information, advocacy, and linking to resources for older adults, caregivers, and persons with disabilities.
Licensed Social Worker with the State of Michigan with Bachelor's degree in Social Work is required; Masters preferred. Formal education with an emphasis on gerontology and two years’ experience working with older adults is strongly preferred. Extensive interviewing and assessment skills and highly developed organizational skills preferred. Excellent verbal and written communication skills and computer/database management skills desired. Knowledge of community resources, especially geriatric and chronic care resources preferred. Reliable transportation and current Michigan driver's license is required. Experience in outreach, marketing and Medicare/Medicaid eligibility preferred.
